Hahahaha no it doesn't atually refer to hitting a nail on the head with a hammer. It means that the nail will ''meet'' someone's head. As in the speaker wants to put a nail into someone's/his own head.
Let`s say you fail miserably in school and get 30% on a very important exam. You could say nail, meet head meaning you would smash a nail into your skull and it would still be less painfull then failing. Or it could be your figurative form of punishment.
